Pacemaker or defibrillator lead in right ventricle
Presence of a transvenous cardiac implantable electronic device (pacemaker or ICD) lead chronically positioned across the tricuspid valve orifice into the right ventricle (apex or interventricular septum).
Definition
Presence of a transvenous cardiac implantable electronic device (pacemaker or ICD) lead chronically positioned across the tricuspid valve orifice into the right ventricle (apex or interventricular septum), traversing the tricuspid coaptation plane and potentially inducing variable degrees of tricuspid regurgitation due to mechanical interference or cicatricial fibrosis without evidence of active infection or lead dysfunction [G7-05] [G7-03].
Severity criteria
- Severity grade: Single grade. Represents the anatomical and physiological baseline state of the intracardiac lead without endocarditis or major prosthetic dysfunction.
-
Quantification of lead-induced tricuspid regurgitation (CIED-induced TR):
- Quantitative criteria for mild tricuspid regurgitation (according to the consensus review of ASE/EACVI and JACC Cardiovasc Imaging [G7-05, Tabla 1] and ESC/EACTS 2025 guidelines [V1, Tabla 9]):
- Effective regurgitant orifice area (EROA): < 0.20 cm² (< 20 mm²).
- Regurgitant volume (RV): < 30 mL/beat.
- vena contracta width (VC): < 3.0 mm.
- Color Doppler jet area: < 5 cm².
- Epidemiological prevalence: New or worsened tricuspid regurgitation jet is documented in 15–40 % of patients undergoing right ventricular lead placement [G7-05].
- Negative criterion: Complete absence of verrucous masses or vegetations attached to the lead and absence of bacteremia or systemic inflammatory response signs [G7-03].

Hemodynamics
Rhythm
ECG
- In sensing mode: Sinus rhythm with normal conduction or baseline intrinsic patient abnormalities [G7-03].
- In pacing mode: Pacemaker spike immediately preceding the QRS complex, with complete left bundle branch block morphology (wide QRS > 120 ms with deep S waves in V1 and slurred R in lateral leads I and aVL) as depolarization originates in the right ventricular apex [E2] [G7-03].
Heart sounds and murmurs
- Tricuspid systolic murmur: Soft or very mild holosystolic murmur (grade 1–2/6) at the lower left sternal border (tricuspid area), or complete absence of murmur if the lead courses commissurally without interfering with coaptation [B1] [G7-05].
- Fundamental sounds (S1 and S2): Preserved and normal; possible wide physiological splitting of S2 during right ventricular pacing due to delayed electrical activation of the left ventricle [B1] [E2].
Arterial pulse
- Peripheral arterial pulse symmetric, rhythmic, and of normal amplitude and morphology [B4].
Jugular venous pulse
- Normal jugular venous pulse with preserved a and v waves and normal physiological x and y descents [B3].
